What is included in "base earnings" as it applies to this program?

Base monthly earnings means your rate of reported monthly earnings on the day before disability begins.

  • For Department of Justice and Homeland Security (DJRA) base earnings include locality pay, scheduled overtime, availability pay, hazard and danger pay for law enforcement officers (if applicable).
  • For Treasury Department and Homeland Security (TDRA) base earnings include locality pay, scheduled overtime, availability pay, hazard and danger pay for law enforcement officers (if applicable).
  • Members of Federal Law Enforcement Officers Association (FLEOA) base earnings include locality pay, scheduled overtime, availability pay, hazard and danger pay for law enforcement officers (if applicable).
  • For Federal Judiciary Employees (NCBC) base earnings include locality pay, scheduled overtime, availability pay, hazard and danger pay for law enforcement officers (if applicable) and net transcript fees for court reporters.
  • For Department of Veterans Affairs (NOVA) base earnings include locality pay, scheduled overtime, availability pay, hazard and danger pay for law enforcement officers (if applicable) performance pay for physicians and dentists, and special or market pay for physicians, dentists and nurse executives.

Excluded are commissions, awards, bonuses, non-scheduled overtime or any other extra compensation.

Where do my employees send their signed form 1199A?

Your employees should complete and sign their Form 1199A, using this form to initiate or change their allotment though Employee Express, Employee Personal Page (EPP) or their payroll servicing personnel. Sending signed Form 1199As to FEDERAL FIRST will delay processing of allotments.

How do my employees convert their coverages?

Employees may convert their coverage if:

  • Their employment ends for a reason other than retirement, or
  • They are no longer in an eligible class: They must apply within 31 days of the termination of group insurance.
  • Please refer to Conversions FAQs for more limitations on conversion coverage.

Eligible employees should complete the following steps to request a conversion of their coverage:

Step 1: Complete and return the Application for Conversion of Group Long Term Disability Coverage form with the first quarterly installment and application fee to:

Step 2: Upon receiving the completed application, JHA will process the request for conversion.
