  • Email us your new address using our Contact Form, or

How do I cancel my coverage?

Upon receipt of your cancellation request a new 1199A Form indicating a $0.00 allotment will be mailed to you along with instructions for processing.

Since you are the only person authorized to change your allotments, your coverage under the program will continue as long as we receive allotments from you. It is important that you process the Form 1199A we send you immediately.

When your allotment stops, your coverage will be canceled effective as of the last day of the pay period for which your last allotment was taken.
