Am I eligible to convert my LTD coverage if I retire from the Federal Government?
No, if you leave your government employment due to retirement you are not eligible for conversion.
Am I eligible to convert my LTD coverage if I leave my job with the Federal Government to pursue employment in the private sector?
Yes, but only if you meet all of the requirements for conversion.
Under what conditions would I not be eligible to convert my FEDERAL FIRST coverage?
The conversion privilege is not available to you if:
- You have not been covered under the group program for at least 12 consecutive months.
- Your insurance under the employer’s group program terminates for any of the following reasons:
- The group program terminates;
- The group program is amended to exclude from coverage the class of employees to which you belong;
- You no longer belong to a class of employees eligible for coverage under the group program;
- You retire (you retire when you receive payment from the employer’s retirement plan as recognition of past services, OR you have concluded your working career.)
- You failed to pay any required premium due.
- You are or become insured for Long Term Disability insurance under another employer’s group insurance program within 31 days after termination.
- You are disabled under the terms of the group program.
- You recover from a disability and you do not return to work for this employer.
- You are currently unable to work due to a mental or physical condition.
- You are on a leave of absence.
How do I request the conversion option?
You must apply within 31 days of the termination of group insurance.
- Return the completed application and submit the first quarterly installment and application fee to Aetna at the following address:

- If you have any questions or need help completing the application please call the JHA Conversion Service Center at 1-888-786-2688 and they will be able to assist you.
- Upon receiving this form, JHA will process your request for coverage.

The coverage under the conversion policy may differ from the FEDERAL FIRST LTD Program by the following:
- Type and amount of coverage provided
- Premium amount
When do my conversion rights expire?
Your application for coverage must be received within 31 days from the date that your employment terminates in order to be eligible.
