How do I file a claim for benefits under FEDERAL FIRST's Long Term Disability Insurance Program?
You can select one of three ways to file a claim for LTD benefits:
You must give Aetna Life Insurance Company (Aetna) written Statement of Claim within 30 days after the first day you miss work due to a disability. When Aetna receives your Statement of Claim, you will be sent forms for providing Proof of Loss within 15 days.

Do I need to wait until after the elimination period to file my claim?
No. A delay in reporting your claim can cause serious delay in payment of benefits.
How long will it take to process my claim?
Disability claims generally take up to 45 days to process.
Will my disability benefit checks be subject to federal income taxes?
No. Your benefits payments will not be subject to federal income taxes since your premiums are paid with after-tax dollars.
How often will I receive a disability benefit check?
Long Term Disability benefits are paid monthly.
Will payment from other sources affect the amount of disability benefits paid to me?
Yes. Payments may be reduced from other sources. Workers’ Compensation, Social Security or the Federal Retirement System benefits are examples.
