Where do I send my signed Form 1199A?
Send your signed Form 1199A to your payroll servicing personnel. If you have access to Employee Express or similar self-service payroll program, you may input / update your allotment information yourself. Returning your signed Form 1199A to FEDERAL FIRST will delay processing of your allotment.
When do I need to change my allotment?
• You receive a cost of living adjustment (COLA)
• You have a birthday that moves you into the next age band
• You receive a promotion, grade or step increase
• You change coverage options (e.g. from Option 30 to Option 90)
What are the allotment/premium age bands?
I just had a birthday, but I'm still in the same age band; does my allotment change?
No. Your allotment only adjusts for age when you move from one age band to the next. For example, a participating employee who turns 35 next year will move into the second age-band and FEDERAL FIRST will send a new Direct Deposit Form 1199A to increase his / her allotment.
My recent birthday moved me into the next age band; how do I change my allotment?
FEDERAL FIRST will automatically send you a new Direct Deposit Form 1199A along with instructions for processing.
How do I advise FEDERAL FIRST of my new income to make sure it is protected?
The easiest way to notify FEDERAL FIRST of your new income is by sending an email to CustomerService@federalfirst.com. BE SURE TO INCLUDE YOUR CERTIFICATE NUMBER INSTEAD OF YOUR SOCIAL SECURITY NUMBER. You may also send a written notice via mail of your new income.
