If I am an eligible employee, when can I enroll?

Eligible employees can enroll for FEDERAL FIRST Voluntary Term Life Insurance coverage, underwritten by Aetna Life Insurance Company (Aetna) at any time. However, an employee who enrolls during their introductory enrollment period has an opportunity to purchase coverage up to the specific guaranteed acceptance limits.

Late enrollees – employees who enroll after their introductory enrollment period – must provide evidence of insurability for any amount of coverage.

How do I change my name?

What are acceptable forms of proof for a name change?

For a name change you can provide one of the following:

  • Drivers License
  • Marriage License
  • Divorce Decree
  • Court Order
  • Personal Action Notice 9A0250 or SF50

How do I cancel my coverage?

Upon receipt of your cancellation request, a new Form 1199A indicating $0.00 will be mailed to you with instructions for processing.

Since you are the only person authorized to change your allotments, your coverage under the FEDERAL FIRST Voluntary Term Life Insurance Program will continue as long as we receive your allotments. For this reason, it’s important that you process the Form 1199A immediately upon receiving it from the FEDERAL FIRST Service Center.

When your allotment stops, your coverage will be cancelled effective as of the last day of the pay period for which your last allotment was taken.
